For more than two decades, Leyline Advocates has provided Case Management Services that help children and adults connect with community resources and coordinated support throughout Boise and the Treasure Valley.

What can be included with Case Management

Case Managers help clients connect with services and coordinate the resources they need. Depending on an individual’s situation, Case Management may include:

  • Connecting with housing resources
  • Connecting with transportation services
  • Connecting with food banks and other community resources
  • Coordinating behavioral health and medical appointments
  • Accessing Medicaid and Social Security resources

Case Management Services may be provided in our Boise office, in a client’s home, or within the community.
